The Community Health Manager oversees and is responsible for implementing and supporting strategies related to State Rural Health Transformation grants. This role focuses on strengthening brain health and care by advancing community‑based engagement efforts across diverse audiences. The Manager will build and leverage state relationships to support and guide the execution of activities that promote scalable, community‑anchored approaches designed to expand access to dementia‑capable care and improve the care experience for individuals living with dementia and their families through the engagement of health and social‑service professionals
