Therapist - Flexible Assertive Community Treatment

Work with others on an inter-disciplinary team, within a Certified Community Behavioral Health Center (CCBHC) model, to provide social services that help clients access the resources they need to recover. Responsible for individual, family and/or group services that will assist individuals in sustaining recovery. Therapists are responsible for providing therapeutic interventions, completing clinical assessments, treatment plan development, and crisis intervention.

Additional services include targeted case management, medication delivery, health promotion, and psychosocial rehabilitation. Services are predominantly delivered to an adult population with a serious mental illness, substance abuse, co-occurring and/or medically co-morbid diagnosis.
