Case Manager, Women's Recovery (Permanent, Full-Time)

The Case Manager is responsible for the women in the Sanctuary Stabilization program from intake to discharge. This position makes the first contact with those who have been referred to the Sanctuary and conducts interviews; provide tours; introduce new participants to the Program; update ShareVision; assist with intake/discharge tasks and orientation. You will create and maintain networks with external partnering organizations, and will regularly attend meetings with Sheway, Richer, FIR Hospital, and other external service providers.
