Youth Care Coordinator - Child Trauma Specialist

$55K annual salary, full benefit package and up to 34 paid days off 1st year! Now accepting applications for a Youth Care Coordinator to provide targeted support services with Youth-centered Systems of Care (SOC). SOC coordinates mental health and community services with the needs of Youth and families impacted by trauma by preventing out-of-home placements and encircling families with support and resources.

Our Care Coordinators work with children in their homes/communities while incorporating the entire family in treatment options. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
