Provide comprehensive care including care coordination and self-management building skills to promote better outcomes for patients. Responsible for compilation of pertinent medical records, comprehensive assessments, care planning, clinical interventions/procedures, patient satisfaction and desired outcomes achieved in the treatment of acute and chronic diseases.

Provide care management and support the patient/family/caregiver prior to treatment, during treatment and following treatment with transitional care and coordination of care as appropriate. This may include specialty care, intermittent hospitalization/ER, community services and support.
