The Care Manager coordinates and links care across agency- and community-based health and social services organizations and provides client advocacy and support. The role involves maintaining a caseload of ongoing clients to provide support, guidance, referrals, and direct services.
Compensation
$60,000/year Shift: Monday-Friday, 8:30am - 5:00pm Union status: This position has the option of becoming a dues-paying member of Service Employees International Union Local 73
