We are seeking a compassionate and dependable Physical Therapist Assistant to join a post-acute care facility in Woodland, California. This role focuses on working with patients recovering from surgery, illness, injury, and medically complex conditions. Job Responsibilities
- Assist the Physical Therapist in implementing individualized treatment plans for post-acute patients
- Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions
- Monitor patient progress and communicate any changes or concerns to the supervising Physical Therapist
- Educate patients and caregivers on prescribed exercises, safety precautions, and mobility techniques
- Maintain accurate and timely documentation of treatments and patient progress
- Ensure compliance with facility policies, safety standards, and regulatory requirements Requirements
- Active state license as a Physical Therapist Assistant in [STATE]
- Current BLS/CPR certification
- Minimum of 6 months of recent PTA experience within the last 5 years preferred
- Experience in post-acute, SNF, or rehabilitation settings is a plus
