Nurse Practitioner - Mon-Fri, 8am-5pm

This is a full-time, salaried, exempt position Schedule: Monday-Friday 8am-5pm

This position works in collaboration with the Medical Director, Hospice Providers, and Community Primary Care Providers to deliver hospice and palliative care services to patients. The nurse practitioner's main duties include serving as the attending provider and performing symptom management and regulatory visits for hospice patients in their homes and extended care facilities, plus other settings as assigned. The nurse practitioner is responsible for developing and implementing a medical plan of care in accordance with the patient’s express wishes and in full compliance with all regulatory requirements including the Nurse Practice Act and agency policy, procedures, and standards of care.
