Bilingual Case Manager Generalist III

A Case Manager, Generalist III provides moderately to highly complex, direct, client-centered case management services and care to patients while seamlessly linking them to medical care and other supportive services at Howard Brown Health. A Case Manager, Generalist III is responsible for working as a part of the Community and Social Health Services, as part of the PCMH model, through screenings and assessments of need, service planning, linkages and referrals to public and private services and programs, information dissemination, advocacy, mental health assessment, service plan follow-up, and discharge planning.
