The Nurse Practitioner provides health education and care within the scope of nursing practice, as a member of client-centered interdisciplinary team, the Nurse Practitioner provides care to individuals promoting wellness, prevention of disease and/or disability and the management of complex chronic diseases/conditions.

The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
