Matthew 25’s Mission: “We exist to serve those at risk for, living with, or impacted by HIV or other STIs through comprehensive healthcare, education, and support while fostering community partnerships to combat stigma and improve overall healthcare and quality of life.” Matthew 25’s Vision: “Our vision is to exist in communities where comprehensive HIV and STI prevention is widespread, ensuring universal access to high-quality, comprehensive healthcare and supportive services.
We aim for those living with HIV to achieve undetectable status, resulting in zero new HIV diagnoses, and to eliminate new STI infections. We are the trusted resource for HIV and STI services, fostering a culture of love, service and hope in every interaction.” Overview: Provides medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP), including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs.
Key Responsibilities
Assess and identify the patient’s individual needs and provide appropriate interventions. Provide advocacy, support, and education to achieve patient wellness. Provide patient-centered approach to access formal and informal supports to develop an individualized care plan and make referrals as necessary. Develop and implement an inclusive individualized care plan.
Encourage health literacy by assisting clients to make informed decisions about their health. Strengthen professional practice by spending time learning best practices in field and develop new skills. Ensure clients obtain certification/recertification for the Kentucky Ryan White Program. Work with patients and multidisciplinary team to ensure the patients have access to medication, treatment adherence counseling, and medical insurance.
Provide appropriate documentation on multiple software platforms. Foster strong connections with clients, informal support, and community partners to ensure the client has access to direct medical care.
QUALIFICATIONS
A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred. Candidates with case management experience of two years or more will also be considered as viable applicants. Experience working in a nonprofit organization or a strong desire to work in service driven environment.
