Job description
Under the general supervision of the Sr Director Provider and Specialty Services, is responsible for working with the primary care teams to integrate behavioral health with medical treatment plans of referred patients. Through psychosocial assessment/intervention appropriate for the diagnosis, the LICSW will assist the patient with achieving an improved quality of life.
Provides crisis intervention, emotional support, and resource linkage in furtherance of optimizing compliance with medical treatment plans and ensuring success with self-management plans.
