**SUMMARY **We are seeking a Patient Care Coordinator to join our team at Alpine Care Home Health. In this role, you will play a vital part in coordinating patient appointments and ensuring the seamless delivery of home health services. Your contributions will directly impact our mission of providing high-quality care that supports individuals in aging with dignity.

RESPONSIBILITIES

The Patient Care Coordinator ensures schedules are prepared to meet demands for service and visit assignments in accordance with the plan of care and state and federal regulations. As a Patient Care Coordinator you will:

  • Serve as the primary point of contact for patients, clinicians, and partner facilities, ensuring professional and effective communication and coordination of care.
  • Support direct care of patients by providing field staff scheduling and adjusting schedules to accommodate physician orders
  • Process referrals and perform insurance verification in a timely and accurate manner.
  • Manually enters visits completed by therapy staff and ensures functional assessments are flagged appropriately
  • Utilize Homecare Homebase (HCHB) efficiently to manage scheduling workflows, patient information, and daily operational tasks.
  • Process therapy reassessment warning and ensure functional assessments are scheduled
  • Be responsible for ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
  • Doing office work requiring sitting, frequent computer and telephone use, and standing, repetitive walking, climbing stairs, bending, and lifting of office materials up to 40 pounds from floor to shoulder. The employee must be able to perform repetitive keyboarding and other administrative tasks.

REQUIREMENTS

  • Currently a Certified Licensed Practical Nurse (LPN) preferred.
  • Proven experience in medical administrative support or scheduling roles.
  • Two years' administrative experience in an office environment, preferably in healthcare
  • Proficiency in Microsoft Office
