Position: Occupational Therapist (OTR) – Skilled Nursing Facility (SNF) We are seeking a licensed Occupational Therapist (OTR) to provide high-quality occupational therapy services in a Skilled Nursing Facility (SNF) setting. The Occupational Therapist will evaluate residents, develop individualized treatment plans, and help improve functional independence, safety, and overall quality of life.
Key Responsibilities
Perform occupational therapy evaluations and develop individualized treatment plans Provide skilled therapy services in accordance with physician orders and facility policies Document evaluations, daily notes, progress notes, and discharge summaries accurately and timely Collaborate with interdisciplinary team members to support resident care plans and goals Educate residents, families, and caregivers on therapy programs and adaptive techniques Maintain compliance with Medicare, Medicaid, and facility documentation standards Participate in care plan meetings and team conferences as required Requirements Qualifications & Requirements: Graduate of an accredited Occupational Therapy program Active Occupational Therapist (OTR) license in the practicing state SNF or long-term care experience preferred (new graduates may be considered) Strong clinical, communication, and documentation skills Ability to manage caseload efficiently and work independently Must meet all facility credentialing and compliance requirements Knowledge of Medicare guidelines and documentation standards preferred Benefits Schedule Options: Full-time / Part-time / Per Diem Weekday and/or Weekend availability Compensation & Benefits: Competitive compensation based on experience Flexible scheduling Supportive clinical and administrative team Assistance with onboarding and credentialing Timely and reliable payroll
