Responsibilities

Perform patient evaluations and physical therapy assessments Review medical history, imaging, and physician referrals Develop individualized treatment plans based on patient needs and goals Guide patients through therapeutic exercises, modalities, mobility training, and rehabilitation programs Educate patients on treatment plans, home exercises, posture, stretching, and lifestyle modifications Monitor patient progress and perform reassessments Maintain accurate and timely EMR documentation Collaborate with physicians and multidisciplinary providers to ensure optimal patient outcomes Requirements Graduate of an accredited Physical Therapy program Active Indiana Physical Therapy license Compensation & Benefits $90K–$105K Base Salary + Monthly Bonuses 2 Weeks PTO/Vacation Continuing Education Reimbursement + License Reimbursement Malpractice Insurance Coverage Mentorship & Professional Growth Opportunities We are looking for a Physical Therapist who is passionate about helping patients live healthier, more active lives in a collaborative and supportive clinical environment.
