Housing Based Case Manager

In collaboration with the County of Marin, housing-based case management goal is to improve the wellbeing of Marin's population who are experiencing chronic homelessness or precariously housed. Two of the program's goals are to provide them navigation support into and ongoing support in permanent supportive housing.
