Counsellor II - Case Manager

The Case Manager is a member of the OMHS multidisciplinary team who supports clients with practical, social, and service-related needs that affect their mental health and overall well-being. Working collaboratively with clients, families, community supports, and the interdisciplinary team, the Case Manager identifies barriers, coordinates services, facilitates referrals, and connects clients to appropriate community resources.

The Case Manager assists clients in navigating health, social, educational, housing, financial, and other relevant systems, and participates in care planning, safety planning, and discharge planning to ensure case management interventions complement the clinical, cultural, and therapeutic supports provided by the OMHS team.
