Intake Specialist - OP Care Management - FT DAYS

POSITION SUMMARY

Responsible for coordinating referrals and providing administrative support to the non-emergency medical transportation department. Reviews data and identifies individuals eligible for services. Visits with eligible patients in the hospital and provides information on the available care outreach services and accompanies community health workers on initial home visits.

Performs a variety of duties that are clerical in nature, including answering phones, scheduling, and preparing charts and client handbooks.
