Hospice Home Team Leader (RN)
- Night Shift Full-Time | 40 Hours Per Week Night Shift | Weekends and Holidays as Needed Leadership Opportunity Lead the Team That Provides Comfort When It Matters Most Exceptional hospice care requires exceptional leadership. What You'll Do
Lead Clinical Operations Supervise, prioritize, and coordinate patient care activities during assigned shifts within the Hospice Home Ensure continuity of care and outstanding patient outcomes Monitor staffing needs and assign admissions appropriately Coordinate care delivery among nursing staff and hospice aides Support interdisciplinary collaboration to achieve patient and family goals Develop and Support Clinical Staff Participate in interviewing, hiring, onboarding, and orientation of new clinical team members Mentor, coach, and support nurses and hospice aides Foster a positive and collaborative team culture Identify staff development opportunities and competency needs Partner with education and quality teams to ensure clinical excellence Ensure Quality and Regulatory Compliance Conduct regular chart audits to ensure documentation accuracy and compliance Monitor compliance with Medicare, Medicaid, ACHC, state, and organizational requirements Review and implement clinical policies, procedures, and best practices Promote quality improvement initiatives and operational excellence Ensure accurate and timely clinical documentation Support Patient and Family Care Partner with patients, families, physicians, and interdisciplinary team members to address care concerns and clinical issues Assist with complex care coordination and problem-solving Ensure compassionate, patient-centered care remains at the forefront of all decisions Manage Operational Responsibilities Oversee medication room operations and regulatory compliance Ensure safe management of narcotics and medications Support scheduling and staffing needs Conduct performance evaluations and personnel management activities Participate in Management Backup (MBU) coverage as assigned Cover at least one holiday annually Who Thrives in This Role Our most successful Hospice Home Team Leaders are: Confident and approachable leaders Strong clinical decision-makers Mentors who enjoy developing others Highly organized and accountable Effective communicators Collaborative team builders Champions of quality and compliance Passionate about hospice care and patient experience Required Qualifications Current Registered Nurse (RN) license in North Carolina Minimum five years of recent clinical experience in hospice, home health, or related healthcare settings Current CPR certification Thorough understanding of hospice clinical interventions and standards of practice Knowledge of Medicare, Medicaid, regulatory, and accreditation requirements Strong leadership, communication, delegation, and organizational skills Computer proficiency and ability to learn new software systems Current unrestricted North Carolina driver's license Reliable transportation Ability to lift up to 30 pounds Preferred Qualifications Previous supervisory or leadership experience Experience managing interdisciplinary teams Hospice inpatient unit experience Experience with performance management, staff development, and coaching Knowledge of clinical quality improvement processes Experience with healthcare documentation and electronic medical records Why This Role Matters The Hospice Home is often where patients and families experience some of life's most significant moments.
