Social Worker

The Outreach Social Worker serves as a Maternal Infant Health Program (MIHP) Care Coordinator who is responsible for coordinating comprehensive, family-centered care for Medicaid-eligible pregnant individuals, infants, and families enrolled in MIHP. The Outreach Social Worker ensures participants receive timely assessments, individualized plans of care, care coordination, education, referrals, and follow-up services in accordance with Michigan Medicaid policy and Michigan Department of Health and Human Services (MDHHS) MIHP requirements.
