Registered Nurse (RN) and Licensed Practical Nurse (LPN) – Start of Care (SOC) & Visits – Brevard County

Matrix Home Care is currently seeking experienced and compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide Start of Care (SOC) assessments and skilled nursing visits throughout Brevard County, Florida. We are looking for clinicians who are passionate about patient-centered care and interested in joining a supportive and growing home health organization.

Responsibilities

• Complete Start of Care (SOC), recertification, resumption of care, and routine skilled nursing visits
• Perform comprehensive patient assessments and documentation
• Develop and implement individualized care plans
• Educate patients and families regarding medications, treatments, and disease management
• Maintain accurate and timely Medicare documentation
• Collaborate with physicians and interdisciplinary healthcare teams
• Ensure compliance with company policies and home health regulations Requirements RN Qualifications:
• Active Florida RN license
• Home health experience preferred
• OASIS documentation experience strongly preferred
• Kan. Time EMR experience preferred
• Valid driver’s license, reliable transportation, and auto insurance required LPN Qualifications:
• Active Florida LPN license
• Home health and skilled nursing visit experience preferred
• Kan. Time EMR experience preferred
