Vermont Comforts of Home (VCOH) offers adults a choice to remain in the community when they are no longer able to live on their own and also require a nursing home level of care. VCOH is a person-centered program. Service Coordinators meet at least monthly with the shared living provider and participant in thei r home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participant’s goals and needs.
Mix of field work and main office in Essex Jct.
