THIS IS A COMMUNITY BASED POSITION, ALL SERVICES RENDERED IN THE COMMUNITY We are seeking a dedicated Qualified Mental Health Professional
- Community Services (QMHP-CS) to provide recovery-oriented services to children, adolescents, and adults. This role focuses on providing high-quality Mental Health Targeted Case Management and Mental Health Rehabilitative Services to help clients achieve stability, develop independent living skills, and improve their overall quality of life. Areas: 77033, 77087, 77547, 77037, 77095, 77096.
Key Responsibilities
Identify, facilitate, and coordinate access to medical, social, educational, and other appropriate services and supports. Skills Training & Development: Provide evidence-based training to help clients develop functional and independent living skills (e.g., symptom management, social skills, and vocational support). Treatment Planning: Collaborate with the clinical team and the client to develop and update individualized Service/Treatment Plans and Crisis Plans.
Assessments: Conduct periodic assessments (such as CANS/ANSA ) to monitor progress and determine continued eligibility for services. Crisis Intervention: Provide immediate support and crisis/emergency services to ensure client safety, including stabilization and linkage to higher care if needed. Documentation: Maintain accurate, timely clinical documentation in the Electronic Medical Record (EMR) system according to Texas Administrative Code (TAC) standards.
Minimum Qualifications
Bachelor’s degree from an accredited college or university in a human services field (e.g., Psychology, Social Work, Counseling, Sociology, Human Development, or Public Health). Note: Candidates with non-human services degrees may qualify with at least one year of documented mental health experience. Certification: Must meet the criteria for QMHP-CS as defined by the Texas Health and Human Services Commission (HHSC).
Skills: Strong interpersonal communication, time management, and the ability to work independently in a community-based or home-based setting. Licensure: Valid Texas Driver’s License and reliable transportation (required for community-based travel).
Preferred Qualifications
Active CANS/ANSA certification. Bilingual (English/Spanish) is highly desirable in the Houston market. Experience with the Texas Resilience and Recovery (TRR) model.
