Job description
This position provides direct patient care to hospice and home health patients in their home or place of residence. The CNA assists in providing a clean and safe environment. They work cooperatively with patients, families, and caregivers and share observations and changes with the RN care manager. They work under the supervision of a Registered Nurse.
