Neuron Health Systems is a community-based provider of acute outpatient rehabilitation and medical and non-medical direct care. We deliver individualized, family-centered support — partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential. The Role We're looking for a Physical Therapist to help patients rebuild strength, mobility, and independence at home and in their community.

This is a hybrid, per-diem role: planning and documentation can largely be done remotely, with in-person visits reserved for evaluations and hands-on treatment sessions. All clinicians are expected to take part in the firm's accreditation activities and surveys as part of maintaining our quality standards.

What You'll Do

Conduct comprehensive physical therapy evaluations, including mobility and home safety assessments. Develop and implement individualized treatment plans. Provide gait training, therapeutic exercise, and fall-prevention strategies. Monitor patient progress and adjust treatment plans as needed. Collaborate with RNs, OTs, SLPs, and other members of the care team.

Complete evaluation, progress, and discharge documentation in a timely manner. Educate patients and family members on home exercise programs and safety precautions. Participate in case conferences and firm accreditation activities and surveys. What You Bring Active NJ Physical Therapist license in good standing. Home care, rehabilitation, or I/DD experience preferred.

Comfort working independently in a remote-first workflow, with reliable transportation for in-person visits. Strong documentation habits and clear communication with patients, families, and the care team.
