Job description
LPN – SNF/LTC JOB SUMMARY: The LPN provides nursing care to residents/patients under the supervision of the Unit Manager/ADON/DON. Responsible for assessing residents, implementing and evaluating care plans, maintaining a safe environment, and supervising nursing assistants and support staff.
KEY RESPONSIBILITIES
Provide nursing care in accordance with established standards and the State Nurse Practice Act. Assess residents and implement/evaluate individualized plans of care. Monitor resident safety and maintain a supportive care environment. Supervise nursing assistants and helpers in resident care activities. Follow facility policies, procedures, and applicable quality standards.
Provide excellent customer service and compassionate resident care. Demonstrate professionalism and teamwork throughout the assignment.
REQUIREMENTS
2+ years of SNF/LTC experience – Required. Active Michigan LPN license – Required. BLS through AHA or American Red Cross – Required. COVID-19 vaccination – Required ; medical/religious exemptions may be considered case-by-case. Previous travel experience – Required. Flexible scheduling required — blocked scheduling cannot be guaranteed.
