Case Coordinator Full-Time

Case Coordinator We are looking for a compassionate and organized Case Coordinator to join our home care team. The ideal candidate is comfortable traveling to client homes and working directly with clients, families, and care providers.

Responsibilities

Conduct client assessments and develop individualized care plans. Coordinate home care services, appointments, and communication with clients, families, and providers. Monitor client progress and update care plans as needed. Maintain accurate documentation Provide emergency direct care coverage when staffing needs arise.

Communicate effectively with clients, families, caregivers, and other service providers.

Qualifications

With healthcare experience; home care or case management experience preferred. Experience working with individuals with disabilities is a plus. Current CPR/First Aid certification. Strong communication, organization, and problem-solving skills. Reliable transportation and willingness to travel to client homes. Ability to work independently and manage multiple client needs.
