LIFE SKILLS COORDINATOR REPORTS TO : Program Director Transitional Living QUALIFICATION S : Must be 2 5 years of age or older Must have a bachelor’s degree from an accredited college or university. A college major in the social science field, psychology, social work, or sociology is considered a plus. Exper ience that includes the following is considered a plus : Providing support services to youth and young adults who may have experienced child abuse and neglect , are in foster care, are homeless, are at risk or have mental or behavioral health challenges.

Providing independent living services to youth and young adults. PRIMARY FUNCTION : Monitor the day-to-day operations of a Supervised Independent Living (SIL) home, serving in the role of a mentor; providing guidance, oversight, and coordination of life skills development opportunities to help residents become more self-sufficient and transition them to independent living.

G ENERAL RESPONSIBLITIES AND DUTIES Develop and implement an age-appropriate Life Skills Transition training program for youth aged 14 through 2 0 who are participants in the Transitional Living program of Hope Haven of East Texas. Monitor and guide participants of Supervised Independent Living (SIL) and the GOH with basic daily living requirements such as nutrition , maintaining scheduling , transportation, housekeeping, financial management , employment, education, personal development and spiritual foundations.

Assist in maintaining records as required by Texas DFPS contract for the SIL program . Collaborate with Hope Haven personnel in facilitating Transition to Adult Living services. Collaborate with Hope Haven personnel in implementing Life Skills plans for all residents aged 14-2 0 . Anticipate and maintain records necessary to track participants' progress in performance objectives established for Transition to Adult Living.

Support the House Manager to ensure meal plans and basic food staples are maintained at the SIL. Monitor and report on SIL residence compliance with Life Skills progress. Consult with superv isor on specific needs of young adults and report al l serious incidents immediately . Serve as a mentor to young adults in the program by being physically present in the home and for an amount of time sufficient to develop a trusting relationship and by being available for the young adults to contact with questions or needs as they arise .

Participate in required training to maintain compliance with Minimum Standards . For this position, thirty hours of job-related training are required for the first year of assignment to child-placing responsibilities. Thereafter, twenty hours of job-relate d training are required . Perform other assignments and duties as requested.
