Job description
City/State: Nyack, New York Department: Pre-Natal Center Work Shift: Day Work Days: DAYS VARY Scheduled Hours: 8:30 AM-5 PM Scheduled Daily Hours: 8.5 HOURS Hours Per Pay Period: 20 Pay Rate/Range: Salary range: $55,000-$65,000 Job Summary Under the direction of the Nurse Manager, the Clinical Nurse Coordinator (CNC), guides, directs, teaches, evaluates and counsels unit staff in providing professional nursing care and complying with established policies and procedures.
She/he acts as a clinical role model implementing the nursing practice model, nursing process and providing direct patient care as needed. The CNC is a professional nurse who assumes accountability for coordination of care for the prenatal center, providing care, managing care, assisting with care and delegating care.
Outcomes management, in accordance with the principles of the care delivery model of the center, is an essential function of the CNC. The CNC is recognized as a professional role model and clinical expert and therefore promotes a professional environment that supports nursing excellence, a professional practice model, and input to the Nurse Manager in reference to hiring, counseling and terminating employees.
Essential Functions
Demonstrates the technical/professional knowledge, skills and abilities necessary for the successful operations of the department including, where applicable, age specific competency. *
- Oversees the daily management of prenatal patient care through the assessment of prenatal patient’s population.
- Rounds with clinicians daily (Nurse Manager, LIP, Techs) to discuss patient’s progress.
- Facilitates direct care when appropriate to complex patients (Problem solving/analysis)
- Assists with facilitation of interdisciplinary team meetings to provide mechanism to collaborate, plan, implement and assess the plan of care
- Trouble shoots complex problems in patient care, documents findings and progress in patient record as needed (Problem solving/analysis)
- Ensures patient/family preparation for care across continuum
- Ensures that patients and family members are provided effective prenatal education to prepare for the birth of a child.
- Ensures a quality experience for all patients of the Prenatal Center.
- Participates in quality patient experience through rounding and observation of staff.
- Supports all organizational initiatives to improve the patient experience
- Maintains knowledge of current trends and developments in nursing practice through a variety of activities including but not limited to reading appropriate literature attending related seminars, and conferences*
- Assists Nurse Manager in projects/assignments/reports as required
- Reviews patient care provisions and outcomes of care. Identifies quality issues and follows through with the nursing staff to improve compliance with accepted standards of care.
- Coordinates Quality Assurance activities.
- Participates in the establishment of annual center goals.
- Ensures compliance with New York State DOH and JCAHO requirements. CUSTOMER SERVICE:

- Demonstrates compliance with licensure/certification requirements and the timeliness of their submission. C. PROJECTS/ASSIGNMENTS:
- Completes projects/assignments within projected time frames and expectations.
Qualifications
Associate Degree Required or Bachelor Degree Required 1 years’ experience in pre-natal clinic preferred Preferred Bi-lingual/fluent in Spanish preferred Registered Nurse New York
- New York State Board of Nursing Required BLS Basic Life Support
