Registered Nurse - Outpatient- Forensic Assertive Community Treatment (FACT)

FACT provides intensive, community‑based services focused on promoting symptom stability, improving coping and social skills, and supporting independent living and community integration. Services include wraparound care coordination, medication management, psychoeducation, substance use counseling, peer support, vocational services, and coordination with community and justice‑system partners.

As a Registered Nurse – FACT, you will be an essential member of this multidisciplinary team, providing person‑centered nursing care, medication support, and health monitoring that improves outcomes for individuals receiving services in the community.
