Activa Home Health is seeking a PRN Physical Therapist for our Medicare and Hospital at Home patients in the Vero Beach, Fort Pierce, Port St. Lucie, Stuart, Indian Town, Hobe Sound and surrounding areas. This role is ideal for PTs with acute care, transitional care, Medicare Oasis knowledge and home health experience who are passionate about restoring mobility, preventing readmissions, and helping patients safely recover at home.
