Job description

​ PURPOSE STATEMENT:  

Plan, manage and coordinate the activities of a patient support team ,   services or programs of the department.

** ​ **** ESSENTIAL FUNCTIONS: **  

  • ​ Coordinate   the work and activities of assigned staff, including setting perf ormance expectations in the day- to - day operations of the shift or unit.  

  • ​ Monitor   performance and   provid e   feedback, coaching, mentoring and training   for   staff.  

  • ​ Respond   to   and manage   emergencies.  

  • ​ Provide staff management to including hiring, development, training, performance management and communication to ensure effective and efficient department operation.  

  • ​ Maintain communication and relationships between medical staff, other department staff and leadership team by attending meetings and coordinating interdepartmental functioning in support of the mission and goals of the organization.  

  • ​ Perform   day-to-day operational and administrative tasks such as maintaining information files and processing paperwork.  

  • ​ Conduct and administer fiscal operations, including planning budgets, authorizing expenditures and coordinating financial reporting.  

  • ​ Prepare activity reports to inform management of the status and implementation plans of programs, services and quality initiatives.  

  • ​ May, as determined by staffing or census needs, assist   patient   with their daily living activities, administer basic health care or treatments, participate in therapeutic or recreational group activities.  

  • ​ Oversee admitting and discharging   of   patient .  

  • ​ Monitor physical and emotional well-being of   patient   and report unusual behavior or physica l ailments to appropriate staff. P repare and maintain records   of progress   and services performed.  

  • ​ Lead or co-lead individual or group sessions as part of specific recreational   or therapeutic procedures   as needed.  

  • ​ Collaborate with other staff in working with the population of the facility to treat, rehabilitate and return   patient   to the community.    

  • ​ Coordinate events   and   activities of others, scheduling program s and activities   as needed.  

  • ​ Provide transportation for   patient   as needed . M ay include driving of facility motor vehicle or coordinating transportation with appropriate staff member.  

  • ​ Demonstrate a positive, empathetic and professional attitude towards customers always.  When patient needs are not met, acknowledge and work to resolve complaints.  Recognize that patient safety is a top priority .  

** ​ **** OTHER FUNCTIONS:   **  

  • ​ Perform   other functions and tasks as assigned.  

** ​ **** EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: **  

  • ​ High S chool   diploma   or equivalent   required.     Associate's   or Bachelor’s   degree in human service field from an accredited college or university preferred.    

  • ​ Must have served in the role of Patient   Support for the facility for one or more   years with an additional   two or more   years of experience in a customer support/service role, preferably in the health or human services field .  

** ​ **** LICENSES/DESIGNATIONS/CERTIFICATIONS:   **  

  • ​ CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).  

  • ​ First aid may be required based on state or facility.  

  • ​ M ay require a v alid state driver's in facilitie s where transporting   patient   is an essential function.
