Continuum of Care Coordinator

Reporting to the Manager of Patient Access, the Continuum of Care Coordinator is committed to providing the highest quality of care as identified in the GHS Mission.

Responsibilities

of this position are to ensure a seamless coordination of patient care from initial referral through any service needed. Acts as the primary in-office customer service contact in the Patient Access Center with physician’s offices and hospital staff to coordinate concierge visits to determine appropriate business lines for patients.
