What You'll Be Doing Provide skilled home health nursing visits throughout the Twin Cities service area Complete comprehensive patient assessments Perform wound care, medication management, injections, and other skilled nursing procedures Educate patients and caregivers on treatment plans and disease management Coordinate care with physicians and interdisciplinary team members Provide coverage across territories to support patient needs and growing census Complete accurate documentation using Homecare Homebase (HCHB) Help ensure continuity of care while delivering exceptional patient experiences Who We're Looking For Active Minnesota Registered Nurse (RN) license Experience in home health, hospital, rehabilitation, skilled nursing, or acute care preferred Strong assessment and critical thinking skills Comfortable adapting to different clinical environments Excellent communication and time management abilities Reliable transportation with the flexibility to travel throughout the Twin Cities Metro About Home Health Care, Inc.
