You'll work closely with clients, medical providers, and community partners to remove barriers and improve quality of life. In this role, you will: Build meaningful relationships with clients Develop individualized care plans and connect clients to resources Coordinate care with healthcare providers and community agencies Support clients in staying engaged in medical care and treatment Assist with benefits, health services, and other support programs Maintain accurate documentation and client records Collaborate with a compassionate, client-centered team Minimum Requirements Bachelor's degree in a health or social service field plus 1-2 years of related experience OR 2-3 years of applicable experience in lieu of a degree Valid Arizona Driver's License/Auto Insurance Bonus Qualifications Bilingual English/Spanish Experience with HIV/AIDS services Knowledge of Ryan White or HOPWA programs Experience with electronic health records Experience in healthcare, social services, behavioral health, housing, or community outreach About Northland Cares Northland Cares is Northern Arizona's leading provider of comprehensive HIV care and support services.

Compensation

& Benefits At Northland Cares, we value the important work our team members do and offer a competitive compensation and benefits package, including: Starting at $22.00 per hour, Candidates with directly related experience, specialized skills, or advanced qualifications may be eligible for a higher starting rate.
