Care Manager, Supporting Our Survivors

Jewish Family Service of San Diego (JFS) is seeking a compassionate, mission-driven Care Manager to join the Supporting Our Survivors (SOS) San Diego team. This full-time Care Manager role supports Holocaust survivors through person-centered, trauma-informed care management, including client intake and assessments, care coordination, crisis intervention, resource navigation, service planning, and ongoing short- and long-term support.

The position also requires accurate documentation and data entry in care management databases to support grant reporting, program outcomes, and quality services.
