Community Health Worker - $2,500 Sign-On Bonus

The Community Health Worker serves as a bridge between patients, healthcare providers, and community resources to reduce barriers to care. This role supports patient-centered care through health education, care coordination, and identification of social factors affecting health, such as housing, food access, and transportation.

The CHW works collaboratively with patients, families, and providers to promote preventive care, manage chronic conditions, and support adherence to treatment plans. Must be CMA certified.
