Case Manager - Targeted Case Management

The Targeted Case Manager provides assessment, monitoring, care coordination, and support services for individuals with mental health needs, including those subject to court-ordered mental health commitments and settlement agreements. The position also provides Targeted Case Management services to individuals who do not require a higher level of care through Comprehensive Community Services (CCS) or the Community Support Program (CSP).

The Targeted Case Manager serves as a liaison among clients, treatment providers, community resources, legal entities, and natural support systems to promote treatment engagement, recovery, safety, and community stability.
