Trustbridge Hospice, a proud member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the western Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday

• Friday 8:30am - 5:00pm with a weekend requirement.
