Registered Nurse (RN) - Home Health (Days)

Home Health Registered Nurse Job Summary Seeking a Registered Nurse to provide comprehensive home health care, coordinate patient services, develop and manage plans of care, and educate patients and caregivers while maintaining compliance with regulatory standards.

Responsibilities

Provide nursing care through the nursing process and maintain professional nursing standards. Supervise CNAs and coordinate care with other healthcare professionals. Complete comprehensive initial and ongoing patient assessments. Assess physical, psychosocial, environmental, and community needs. Initiate, evaluate, and modify Plans of Care as needed.

Establish discharge plans with patients, physicians, caregivers, and other disciplines. Complete OASIS assessments and required documentation. Provide patient care in accordance with Federal, State, and agency regulations. Assess patient and caregiver education needs and provide appropriate teaching. Maintain accurate documentation according to regulatory requirements.

Promote a clean and safe patient care environment. Maintain patient privacy and confidentiality.

Requirements

Home Health experience required. Active BLS or willingness to obtain required. Valid driver's license and auto insurance required. Active Illinois nursing license required. Strong Med/Surg experience with some Home Health experience may be considered. OASIS and Start-of-Care experience strongly preferred. COVID and Flu vaccination required; declinations will not be approved.
