Case Manager- Wayne County

Catholic Charities, Diocese of Cleveland is seeking a case manager to provide direct service to individuals in the WISE Program in Wayne & Holmes Counties. The WISE Program serves individuals with a developmental disability and mental health challenge. The staff member will be trained in Wraparound facilitation. The staff member will provide support, linkages to services and reinforce treatment interventions identified by the client's therapist.
