Springpoint at Home is seeking an experienced Care Management Coordinator to join our team!

Springpoint at Home offers home care and care management services to residents of our Life Plan Communities as well as to individuals that live in their private home. We are seeking a full-time Care Management Coordinator/Aging Life Care Advisor to join our team. At Springpoint, our CMC is a health and human services professional who acts as a guide and advocate for their clients and/or families as well as all Care Management employees. The CMC will maintain a specialized focus on issues related to aging and elder care and is responsible for daily Care Management and activity.s).
