Education

•Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience. Licenses and Certification •Illinois license in Physical Therapy •BLS Experience and Skills •Technical Experience: N/A Role Specific Responsibilities •Performs thorough evaluation •Develops appropriate Plan of Care •Implements Plan of Care •Documentation is accurate, complete and timely •Provides supervision to assistants, rehab techs, and students Compensation (Commensurate with experience): $41.
