Intensive Case Management Worker - HOPE

The Intensive Case Management worker works collaboratively as part of a multi-disciplinary team that provides services to participants who are, or at risk of being homeless and who are unable to utilize their natural support systems. Direct service provision includes service planning, goal planning, direct life skills teaching, advocacy, linkage, and expanding support networks.

The position is responsible for maintaining and managing the Service Plan and is expected to ensure open and accurate communication with all staff and community stakeholders regarding Participant needs, setbacks, and successes. Will work closely with Participant Support Worker(s) to assist in meeting the needs of program participants, supporting health, safety responsibilities and other daily tasks as required.
