RN Coordinator of Care Peds

Job Description: Position : Visiting Nurse ( RN Coordinator of Care) The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This would include Conducting home visits, completion of all required documentation and communication with PCP.

Full time schedule : Monday- Friday Full Time RN Benefits : Sign on Bonus Tuition Reimbursement annually 401K plan with company match. 4 weeks of vacation, 5 personal days, and 8 sick days per year. Monthly bonuses, based on personal productivity. Paid orientation in the office and in the field. Company tablet with software and technical support team provided.

Requirements

NYS RN License We welcome New Grads, busy parents, retirees, hospital nurses looking for a change in pace! Valid driver’s license and reliable transportation. Strong ability to work independently with minimal supervision Excellent clinical skills Basic knowledge of office applications and equipment (Outlook, Word, Printing and scanning applications).
