Baptist Health is seeking a dedicated and compassionate Registered Nurse to join our Case Management team. This is a PRN day shift opportunity and plays a key role in supporting patients throughout their hospital stay by ensuring coordinated, high-quality care.

Registered Nurse, Case Management, Job Responsibilities Provide comprehensive care coordination for hospitalized patients. Conduct high-risk screenings and assessments to identify patient needs. Arrange and document post-acute care services , ensuring smooth transitions of care. Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.

Experience Requirements: 1 year case management experience Education & Credential Requirements: Associate of Science in Nursing required Bachelor of Science in Nursing preferred Cardiopulmonary Resuscitation Certification (CPR) Required Licensed Registered Nurse Required If you are a motivated RN with strong communication and care-coordination skills, we encourage you to apply for this rewarding opportunity located at Baptist South!
