Empath Trustbridge Hospice is currently seeking a caring and motivated RN Hospice Case Manager to join our team in Palm Beach County. If you are passionate about patient-centered care and want to make a profound impact in the lives of patients and families, this is your opportunity. You would work the following schedule: Monday - Friday 8:30am - 5:00pm with an every other weekend requirement
