Case Manager - Behavioral Health ICMS

This position of Intensive Case Manager for Permanent Housing provides targeted services to the most vulnerable populations in Los Angeles County by employing a “whatever it takes” approach to assist clients with transitioning from homelessness to permanent housing. Clients served will be homeless or very recently housed, have a chronic illness or disabling condition, and are high utilizers of public services.

The Intensive Case Manager for PH shall serve as the central point of contact for the referred client. Case management services may range from highly intensive individualized support to less intensive support for activities related to acquiring and maintaining housing, and associated supportive services.
