Job description

Perform a thorough assessment of functional ability related to behaviors of concern, emotional and social needs, the physical and social environment in which consumers work and reside, the relationships individuals have with their parents, staff, housemates, and family, as well as the individual's physiology.
Develops support plans (e.g., direct individual or group therapy, skill acquisition plans, staff support plans, preventative strategies, etc.) that facilitate a feeling of being valued, the establishment of meaningful personal relationships, the ability to make meaningful choices based on life experiences, and the acquisition of and the ability to practice new skills
