Job description
The Coordinated Entry Outreach Facilitator is responsible for the intake of at risk youth in the housing services program by completing SPDATs. The facilitator will support the youth in a warm transfer to housing. They will also help the youth to access appropriate community support, job skill development, drug counselling, legal assistance and education support.
The Coordinated Entry Outreach Facilitator will work closely with the Arcadia Housing Team at Mc. Man and support the team. The Coordinated Entry Outreach Facilitator is will also work to promote the program in the community.
